Lurbinectedin as second‑line treatment for patients with small‑cell lung cancer: a single‑arm, open‑label, phase 2 basket trial
Sacituzumab govitecan shows promising efficacy as second‑line treatment in extensive‑stage small cell lung cancer, yielding a 41.9% objective response rate and a median overall survival of 13.6 months, irrespective of platinum sensitivity.
This multicohort, open‑label, phase‑2 basket study (NCT03964727) evaluated sacituzumab govitecan (10 mg/kg on days 1 and 8 of a 21‑day cycle) in 43 adults with extensive‑stage small cell lung cancer that progressed after one line of platinum‑based chemotherapy and anti‑PD‑L1 therapy. Investigator‑assessed objective response rate was 41.9% (95% CI 27.0‑57.9%, 18 confirmed partial responses). Median duration of response, progression‑free survival and overall survival were 4.73 months, 4.40 months and 13.60 months, respectively. The benefit was similar for platinum‑resistant and platinum‑sensitive disease. Toxicity was manageable, with 74.4% experiencing grade ≥ 3 treatment‑emergent adverse events and no treatment‑discontinuing events except one neutropenic sepsis that led to death.
The results indicate a clinically meaningful response rate and survival advantage in a setting where therapeutic options are limited after first‑line therapy, providing a potential new standard for second‑line treatment of extensive‑stage SCLC.
