The 2026 RISE UP for Breast Cancer and Women9s Health Conference presented early results from the PRSONAL randomized clinical trial. The study evaluated whether individualized breast cancer risk calculated with the CanRisk model could safely determine screening intervals and whether communicating that risk had psychological benefits. Women aged 50‑67 were randomized to continued standard biennial screening or to risk-based screening where ten‑year absolute risk was computed and communicated automatically. The trial assessed clinical outcomes (detection, recall, interval cancers) and psychological impact (distress, acceptance of de‑escalated screening). Findings demonstrate that a risk‑stratified approach can be embedded into routine national programs and have implications for tailoring screening frequency to individual risk, potentially improving benefit‑to‑harm ratios.
Traditional uniform mammography screening often over‑treats low‑risk women and under‑treats high‑risk women. Evidence that risk‑based screening is feasible and psychologically acceptable supports a shift toward personalized screening schedules, which could enhance mortality benefits while reducing unnecessary procedures and anxiety.
