20‑Year Risks of Breast‑Cancer Recurrence after Stopping Endocrine Therapy at 5 Years

Risk of breast‑cancer recurrence continues steadily up to 20 years after stopping 5‑year adjuvant endocrine therapy, with the absolute risk strongly correlated to the original nodal and tumor size status.

In a meta‑analysis of 88 randomized trials involving 62 923 women with ER‑positive, early‑stage breast cancer who had been scheduled to receive 5 years of adjuvant endocrine therapy and then discontinue, the incidence of distant recurrence, any breast‑cancer event, and breast‑cancer death from 5 to 20 years after cessation was evaluated. The absolute risk of distant recurrence ranged from 10 % in T1N0‑low‑grade disease to 41 % in T2N4‑9 disease, with the pattern of risk being strongly driven by the original tumor size and nodal status. Distant recurrence was steady throughout the 15‑year follow‑up, providing evidence that risk continues beyond the initial treatment period. These data inform decisions about extending endocrine therapy beyond 5 years.

The findings help clinicians and patients weigh the benefits of continued endocrine therapy against its side effects by quantifying the long‑term risk of recurrence after stopping treatment, thus supporting personalized post‑treatment management.
