Metabolic predictors of response to immunotherapy in locally advanced or metastatic esophageal cancer: unveiling the obesity paradox

Rákkutatás · Esophageal cancer

Metabolic pressure from BMI and MetS are independent prognostic factors for survival and therapeutic response in esophageal cancer.

The study evaluates how body‑mass index (BMI) and metabolic syndrome (MetS) influence survival and response to PD‑1 inhibitors in patients with locally advanced or metastatic esophageal cancer (primarily squamous cell carcinoma). High BMI or MetS is associated with better overall survival, progression‑free survival and objective response rates, likely due to a more favorable immune profile (lower pan‑immune inflammation value and higher combined positive score). A nomogram combining gender, BMI, TNM stage and PIV was built and validated internally and externally.

Identifying metabolic predictors can help stratify patients who are more likely to benefit from PD‑1 blockade and guide personalized treatment decisions in esophageal cancer.
