This large-scale study based on real-world data confirms the previous finding that CML patients reach similar survival as the general population but also raises a major concern about the development of comorbidities and poorer quality of life after the diagnosis of CML.

A global retrospective cohort study of 6,000+ chronic myeloid leukemia (CML) patients treated with tyrosine‑kinase inhibitors (TKIs) across 57 health‑care organisations used TriNetX real‑world data. The analysis examines survival, cardiovascular events, second malignancies and other comorbidities, comparing CML patients to matched non‑CML cohorts and evaluating the impact of first‑line TKI choice. Results indicate overall survival in CML is comparable to the general population, but CML patients have a markedly higher incidence of second malignancies and cardiovascular disease, with no significant difference among imatinib, dasatinib, or nilotinib. Reduced adverse events after TKI discontinuation are emphasized, underscoring the value of real‑world evidence to confirm clinical trial data and guide practice globally.

Real‑world data validates clinical trial survival outcomes in CML and highlights increased risks of secondary cancers and cardiovascular disease, informing treatment decisions, surveillance strategies, and patient counselling worldwide.
