Nimotuzumab Plus Gemcitabine for K‑Ras Wild‑Type Locally Advanced or Metastatic Pancreatic Cancer

Nimotuzumab plus gemcitabine significantly improves overall survival and progression‑free survival with a good safety profile in patients with locally advanced or metastatic K‑Ras wild‑type pancreatic cancer.

This multicenter, randomized, double‑blind, Phase III trial evaluated nimotuzumab plus gemcitabine versus placebo plus gemcitabine in 82 patients with locally advanced or metastatic K‑Ras wild‑type pancreatic cancer. Patients received weekly nimotuzumab (400 mg) or placebo with standard gemcitabine. Median overall survival was 10.9 months versus 8.5 months (HR 0.66; 95% CI 0.42‑1.05) and median progression‑free survival was 4.2 months versus 3.6 months (HR 0.60; 95% CI 0.37‑0.99). Restricted mean survival analyses showed significant benefits with nimotuzumab. Objective response and disease‑control rates were comparable between arms; adverse events were similar. The study concludes that nimotuzumab plus gemcitabine improves survival outcomes with acceptable safety.

Identifying K‑Ras wild‑type pancreatic cancer allows the use of nimotuzumab to improve survival, demonstrating the clinical relevance of molecular subtyping in this deadly disease.
