Successful management of metachronous dual primary esophageal carcinomas: first report of consecutive adenoid cystic carcinoma and squamous cell carcinoma with exceptional treatment outcome – a case report
This case illustrates that SPMs may arise outside irradiated regions after ACC treatment, excluding radiation‑induced carcinogenesis.
A 58‑year‑old male with esophageal adenoid cystic carcinoma (ACC) underwent surgical resection followed by adjuvant radiotherapy (50 Gy/25 fractions). Sixteen months later, a metachronous cervical esophageal squamous cell carcinoma (SCC) was detected outside the prior radiation field and was successfully treated with definitive chemoradiotherapy (60 Gy/28 fractions with concurrent S‑1). At March 2026 follow‑up, the patient remained disease‑free.
The report demonstrates that a metachronous second primary esophageal cancer can be effectively managed with curative intent even after prior radiotherapy, highlighting the importance of comprehensive surveillance and multidisciplinary treatment planning for rare esophageal malignancies.
