Five-Year Data Support Neoadjuvant THP in ERBB2-Positive Breast Cancer

Neoadjuvant THP (paclitaxel + trastuzumab + pertuzumab) delivers excellent long‑term event‑free survival and recurrence‑free intervals with near‑universal ctDNA clearance in early‑stage ERBB2‑positive breast cancer.

In a prespecified secondary analysis of the phase‑2 DAPHNe trial, 98 patients with stage II‑III ERBB2‑positive breast cancer received 12 weeks of neoadjuvant THP. Five‑year follow‑up (median 5.2 yr) showed 99 % event‑free survival, 98 % recurrence‑free interval, and 99 % overall survival. Ultrasensitive ctDNA was detected in 89.5 % at baseline and cleared in 96.1 % after therapy, remaining <10 % post‑operatively. The data suggest superior long‑term efficacy of THP and potential prognostic value of ctDNA clearance.

HER2‑positive early‑stage breast cancer remains a high‑risk subgroup; demonstrating durable outcomes with neoadjuvant THP supports its use as a preferred frontline therapy and highlights ctDNA as a minimally invasive marker for therapeutic monitoring and relapse risk assessment.
