Advancements in Endometrial Cancer Research in 2024
Clinical evidence from Phase III trials RUBY Part 1, NRG‑GY018 and DUO‑E demonstrates that combining immunotherapy (dostarlimab, pembrolizumab, durvalumab) with carboplatin‑paclitaxel chemotherapy improves progression‑free and overall survival, especially in patients with mismatch‑repair deficient (dMMR) disease.
Endometrial cancer is one of the most common gynaecological cancers in high‑income countries and, for many years, has been associated with poor outcomes for patients with advanced‑stage disease. The results from the Phase III studies, RUBY Part 1 with dostarlimab, NRG‑GY018 with pembrolizumab, and DUO‑E with durvalumab, in patients with advanced or recurrent endometrial cancer in 2023 were practice‑changing. The addition of immunotherapy to carboplatin‑paclitaxel chemotherapy, followed by immunotherapy maintenance, led to a marked improvement in progression‑free survival (PFS), particularly in patients with mismatch repair deficient, microsatellite stability‑high (dMMR/MSI‑H) disease, and has transformed the treatment landscape for these patients.
The integration of immunotherapy into first‑line chemotherapy has become the new standard of care for advanced or recurrent endometrial cancer, especially for patients with dMMR/MSI‑H disease, offering the possibility of durable responses and improved survival.
