Selinexor Maintenance Improves PFS in TP53 Wild-Type Endometrial Cancer
Selinexor maintenance therapy significantly prolongs progression‑free survival in patients with TP53‑wild‑type endometrial cancer.
In the phase 3 SIENDO trial, selinexor maintenance therapy yielded a median PFS of 28.4 months versus 5.2 months with placebo (HR = 0.44, P = .0005) in the TP53‑wild‑type subgroup. In the subset of patients with proficient mismatch repair (pMMR), median PFS was 39.5 months versus 4.9 months with placebo (HR = 0.36, P = .0011). These findings demonstrate a statistically and clinically meaningful improvement in disease control for this high‑risk population.
Improved PFS in TP53‑wild‑type endometrial cancer represents a critical advance, offering a new maintenance option that may delay recurrence, improve quality of life, and potentially extend overall survival in a setting where therapeutic choices are limited.
