History of the early development of aromatase inhibitors for treatment of breast cancer

Rákkutatás · Breast cancer

These studies provided definitive evidence of the superiority of aromatase inhibitors over tamoxifen in postmenopausal women with hormone responsive breast cancer.

The early development of aromatase inhibitors for the treatment of hormone‑responsive breast cancer was guided by the Penn State Hershey collaborative group. Initial observations with aminoglutethimide (AG) combined with hydrocortisone (HC) demonstrated significant tumour regression in post‑menopausal women. Subsequent mechanistic studies confirmed that AG blocked aromatase activity by 95–98 %, establishing it as the first selective aromatase inhibitor. Randomised controlled trials showed that AG/HC medical ‘adrenalectomy’ was not inferior to surgical adrenalectomy or to tamoxifen, proving the superiority of aromatase inhibition in this patient population.

The demonstration that aromatase inhibitors outperform tamoxifen established a new standard of care for post‑menopausal women with estrogen‑receptor‑positive breast cancer, leading to widespread clinical adoption of these agents.
