Toward Targeted Treatment Strategies for Mantle Cell Lymphoma
Results of brexucabtagene-autoleucel for patients with relapsed/refractory Mantle Cell Lymphoma in the routine setting in Germany and Switzerland
Patients with mantle cell lymphoma (MCL) could benefit from additional safe and effective treatment options. A phase I/II study (Dana‑Farber 21‑040; NCT04855695) evaluated the safety and activity of a time‑limited, chemotherapy‑free regimen combining the Bruton tyrosine kinase inhibitor (BTKi) acalabrutinib, the Bcl‑2 inhibitor venetoclax, and the CD20 monoclonal antibody obinutuzumab (AVO) in individuals with relapsed and refractory (R/R) and treatment‑naïve (TN) MCL. Initial results presented at the 2025 American Society of Hematology (ASH) Annual Meeting showed an overall response rate (ORR) of 88% and a complete remission (CR) rate of 83% among 24 high‑risk TN MCL patients, with a 2‑year progression‑free survival (PFS) of 79% at a median follow‑up of 24 months. Low‑risk TN MCL patients achieved 100% ORR, CR, and 1‑year PFS at 13 months median follow‑up. Grade 3‑4 neutropenia occurred in 30% of participants; infections were observed in 32%. These findings support expansion of the high‑risk TN cohort and parallel investigations into biomarkers of response and resistance.
The study demonstrates that a targeted, chemotherapy‑free triplet regimen can achieve high response rates and durable remission in both high‑risk and low‑risk treatment‑naïve mantle cell lymphoma patients, offering an alternative with potentially better tolerability than conventional chemoimmunotherapy.
