Oncocytic adrenocortical carcinoma: A case report

Complete surgical resection remains the cornerstone of treatment for oncocytic adrenocortical carcinoma (OACC).

Oncocytic adrenocortical carcinoma (OACC) is an exceptionally rare adrenal malignancy. We report a case of a 47‑year‑old male in whom pre‑operative CT/MRI revealed a left adrenal bed mass, with the largest lesion measuring 11.6 cm and suggestive of renal invasion. All other pre‑operative hormone test results were within normal limits, except for the adrenocorticotropic hormone test. The patient underwent open surgical resection with partial nephrectomy. Histopathology confirmed OACC according to the Lin‑Weiss‑Bisceglia criteria. Complete surgical resection remains the cornerstone of treatment.

This case illustrates the diagnostic and therapeutic challenges posed by the exceedingly rare oncocytic variant of adrenocortical carcinoma, highlights the importance of complete surgical resection, and underscores the need for greater awareness of its presentation in patients with prior adrenal surgery.
