Fertility-preserving treatment of endometrial cancer and endometrial atypical hyperplasia for patients with metabolic abnormalities: Challenge or opportunity?
The study examined the impact of combining anti‑estrogen therapy with metformin on the fertility‑preserving treatment of endometrial cancer.
A retrospective analysis of 285 endometrial cancer and endometrial atypical hyperplasia patients treated between 2018 and 2023 evaluated the efficacy of anti‑estrogen therapy combined with metabolic‑targeting interventions (metformin, statins, calcium‑channel blockers). The combination therapy did not improve complete‑response rate overall, but metformin significantly increased complete‑response rates in patients with insulin resistance and diabetes mellitus.
Fertility‑preserving treatment of endometrial cancer requires effective therapies that maintain ovarian function while achieving disease control. The impact of metabolic‑targeting agents such as metformin on progestin‑informed endocrine therapy may influence both response rates and time to response. Understanding these effects ensures that clinicians can appropriately weigh the benefit of adding a metabolic‑targeting agent to an endocrine regimen in the population of patients with endometrial cancer who wish to preserve fertility.
