Effect of Total Laparoscopic Hysterectomy vs Total Abdominal Hysterectomy on Disease-Free Survival Among Women With Stage I Endometrial Cancer: A Randomized Clinical Trial

Between October 7, 2005, and June 30, 2010, the Laparoscopic Approach to Cancer of the Endometrium (LACE) trial randomized 760 women with stage I endometrioid endometrial cancer to total abdominal hysterectomy (TAH) or total laparoscopic hysterectomy (TLH); the disease‑free survival rate at 4.5 years was 81.3% in the TAH group and 81.6% in the TLH group, demonstrating equivalence.

This trial establishes that a less invasive surgical approach (TLH) provides disease‑free outcomes equivalent to the standard open surgery (TAH) for stage I endometrial cancer, supporting a shift toward minimally invasive management.
