Nivolumab-Based Treatment Yields Long-Term Survival in Resectable NSCLC

Perioperative nivolumab treatment demonstrated long-term survival among patients with resectable stage IIA‑IIIA non‑small cell lung cancer (NSCLC), especially among those with a pathological complete response, according to a phase 2 trial (NCT04015778). After a median follow‑up of 50.5 months, the event‑free survival (EFS) and overall survival (OS) rates for the intent‑to‑treat population were 57.2 % and 72.1 %, respectively. In patients with a pathological complete response (pCR), 4‑year EFS was 79.7 % and OS was 94.1 %. The study also identified that ≥13 cycles of adjuvant nivolumab correlated with improved 3‑year EFS (79 % vs 60 %) and suggested that LAMPAD‑low‑risk patients may be candidates for treatment de‑escalation.
This finding supports the use of perioperative nivolumab as a standard of care for early‑stage NSCLC, potentially reducing the need for extensive adjuvant therapy in patients identified as low risk.
