Regorafenib Improves Progression‑Free Survival in Metastatic Osteosarcoma
Regorafenib significantly improves progression‑free survival in patients with progressive metastatic osteosarcoma.
The SARC024 phase II trial evaluated the multikinase inhibitor regorafenib versus placebo in patients with progressive metastatic osteosarcoma. Forty‑two patients were enrolled from 12 centers and randomized 1:1. Median PFS was 3.6 months with regorafenib compared to 1.7 months with placebo (HR = 0.42, 95% CI 0.21‑0.85; P = .017). Overall survival did not differ significantly due to early crossover and limited sample size. Grade 3‑4 events occurred in 64% of regorafenib patients, including one colonic perforation. The study met its primary endpoint and supports regorafenib as a treatment option for relapsed metastatic osteosarcoma.
Osteosarcoma has limited effective therapies after relapse; regorafenib offers a clinically meaningful improvement in progression‑free survival and may expand treatment options for this aggressive malignancy.
