Long‑term survival with nivolumab in a patient with renal cell carcinoma: a case report and review of the literature

A heavily pre‑treated patient with metastatic renal cell carcinoma achieved durable complete remission after 48 months of third‑line nivolumab therapy.

Immunotherapy is the standard of care for advanced renal cell carcinoma (RCC). We report a 67‑year‑old man with stage IV ccRCC who, after two prior systemic lines (sunitinib, everolimus), received third‑line nivolumab (240 mg every 2 weeks). Fifteen months later he achieved a partial response; 24 months of treatment led to complete resolution of all pulmonary metastases, and he remains in complete remission after 48 months of therapy.

The case demonstrates that nivolumab can produce long‑term, durable remissions even beyond the second line in heavily pre‑treated metastatic RCC, supporting its use in later clinical settings.
