Colorectal Cancer Screening: An Updated Modeling Study for the US Preventive Services Task Force

This microsimulation modeling analysis suggests that screening for colorectal cancer with stool tests, endoscopic tests, or computed tomography colonography starting at age 45 years provides an efficient balance of colonoscopy burden and life-years gained.

The US Preventive Services Task Force (USPSTF) updated its colorectal cancer screening recommendations by conducting an updated microsimulation modeling study. Using three independent microsimulation models, the study evaluated a broad range of screening strategies, including fecal immunochemical tests, multi-target stool DNA tests, flexible sigmoidoscopy, colonoscopy, and computed tomography colonography, across various start and stop ages and testing intervals. The results showed that beginning screening at age 45 provides a more efficient balance of life‑years gained versus colonoscopy burden compared with starting at 50, supporting a lower age of initiation. These findings inform evidence‑based guideline updates.

Screening for colorectal cancer has been shown to reduce incidence and mortality, but the optimal age to start and the most cost‑effective strategies remain uncertain. This modeling study provides long‑term outcome estimates that help the USPSTF and clinicians choose screening protocols that maximize benefit while minimizing burden and harm, potentially leading to earlier detection and improved survival.
