Role of Radiotherapy in the Multidisciplinary Treatment of Lacrimal Gland Adenoid Cystic Carcinoma

Rákkutatás · Adenoid cystic carcinoma

The addition of radiotherapy significantly improved the 5-year RFS (63.4% vs 31.1%, p = 0.014) and PFS (56.8% vs 31.1%, p = 0.036) while showing no improvement in OS (79.7% vs 85.6%, p = 0.98) and DMFS (67.3% vs 70.1%, p = 0.70) compared to surgery alone.

This retrospective cohort study evaluated the impact of postoperative radiotherapy (PORT) on overall survival (OS), progression‑free survival (PFS), recurrence‑free survival (RFS), and distant metastasis‑free survival (DMFS) in 90 patients with lacrimal gland adenoid cystic carcinoma (LGACC). Patients treated with surgery alone were compared to those receiving surgery plus PORT. PORT significantly improved 5‑year RFS (63.4 % vs 31.1 %, p = 0.014) and PFS (56.8 % vs 31.1 %, p = 0.036) but did not enhance OS (79.7 % vs 85.6 %, p = 0.98) or DMFS (67.3 % vs 70.1 %, p = 0.70). Proton/carbon‑ion therapy did not differ from photon therapy in any survival endpoint. Radiotherapy was identified as a protective factor against local recurrence in logistic regression analysis.

Post‑operative radiotherapy enhances local disease control (RFS) and disease‑free interval (PFS) without increasing overall survival, informing clinicians that PORT can reduce recurrence risk while tolerable toxicity is a key consideration in LGACC management.
