HER2-Targeted Therapies in Metastatic Colorectal Cancer: A Systematic Review and Meta-Analysis
HER2‑targeted therapies (HER2Tx) demonstrated efficacy in pretreated metastatic colorectal cancer (mCRC) patients, with trastuzumab‑deruxtecan (T‑DX) and trastuzumab‑tucatinib (T‑Tu) showing higher response rates and disease control compared to trastuzumab‑pertuzumab (T+P).
HER2 amplification identifies a subgroup of colorectal cancer with poorer prognosis and resistance to anti‑EGFR therapy. This systematic review and meta‑analysis evaluated phase II/III clinical trials of HER2‑targeted therapies (HER2Tx) in HER2‑overexpressed metastatic colorectal cancer (mCRC) patients. Ten clinical trials (total 467 patients) reported objective response rate (ORR) and disease control rate (DCR). Pooled ORR was 33.7 % (29.6‑38.1 %) and DCR 68.5 % (58.1‑77.4 %). Median overall survival (OS) was 13.4 months, and median progression‑free survival (PFS) 5.5 months. Subgroup analyses showed higher ORR and DCR for trastuzumab‑deruxtecan (T‑DX) and trastuzumab‑tucatinib (T‑Tu) compared to trastuzumab‑pertuzumab (T+P).
HER2‑amplified metastatic colorectal cancer is often resistant to anti‑EGFR therapies; effective HER2‑targeted treatments could improve response rates, disease control, and survival, thereby addressing an unmet clinical need.
