Metastatic pattern and prognosis of gastrointestinal stromal tumor (GIST): a SEER-based analysis
Patients with metastasis had a poorer prognosis than those without.
This retrospective SEER‑based cohort study evaluated the relationship between liver, bone and lung metastases in 4,224 gastrointestinal stromal tumor (GIST) patients diagnosed between 2010 and 2015. The overall prevalence of liver, bone and lung metastasis was 9.19 %, 0.47 % and 0.76 %, respectively. Median overall survival (OS) was 49 months for liver metastasis, 18 months for bone metastasis and 20 months for lung metastasis, all significantly shorter than in non‑metastatic patients. Multivariate analysis identified age < 65 years, female sex, married status and receipt of surgery as independent factors improving OS in patients with liver metastasis. The study concludes that liver metastasis is the most common and prognostically significant site of spread in GIST, warranting focused monitoring.
Understanding metastatic patterns in GIST informs surveillance strategies, prognostication, and therapeutic decision‑making. Early detection of liver involvement, which drives survival outcomes, can guide imaging protocols and surgical or systemic therapy selection, ultimately improving patient management.
