‘STARPORT’ trial probes new approach for aggressive prostate cancer
Adding PET‑directed local therapy (surgery or radiation) to standard systemic therapy may improve outcomes in men with oligometastatic prostate cancer.
The VA STARPORT trial is a multi‑site randomized trial evaluating whether PET‑guided local therapy (surgery or radiation) added to standard systemic therapy (primarily hormone therapy) improves survival, disease control and quality of life in veterans with oligometastatic prostate cancer, defined as five or fewer metastatic sites. The trial plans to enroll about 460 men across 16 VA sites, randomizing them to either standard systemic therapy alone or to the same systemic therapy plus PET‑directed local treatment. Participants will be followed for five years with serial imaging, PSA monitoring and quality‑of‑life assessments. Genomic profiling from tumor tissue will be performed to identify mutations that might predict benefit from the local approach.
Prostate cancer that has metastasized is usually treated only with systemic therapy, which slows but does not eradicate disease. If a PET‑guided focused local treatment can reduce or eliminate the limited metastatic lesions, it could enhance long‑term survival, reduce toxicity and preserve patients’ quality of life.
