Induction vs Adjuvant Chemoradiotherapy in Patients With High-Risk N2 to N3 Nasopharyngeal Carcinoma
Induction‑concurrent chemotherapy did not significantly improve progression‑free survival compared with concurrent‑adjuvant chemotherapy in high‑risk nasopharyngeal carcinoma.
In this phase 3 randomized trial of 324 patients with high‑risk N2‑N3 nasopharyngeal carcinoma, induction‑concurrent chemotherapy did not improve 3‑year progression‑free survival compared with concurrent‑adjuvant chemotherapy. Both regimens yielded similar local control, while adjuvant gemcitabine‑cisplatin was superior to adjuvant cisplatin‑fluorouracil for distant control. Toxicities were comparable, with two treatment‑related deaths in the induction arm.
The result informs clinicians that adding induction chemotherapy does not confer a survival advantage over adjuvant chemotherapy, guiding treatment sequencing decisions for patients with advanced nasopharyngeal carcinoma.
