R‑CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) is the first‑line therapy for most patients younger than age 80 years with DLBCL of any stage.

Diffuse Large B‑Cell Lymphoma (DLBCL) is the most common subtype of non‑Hodgkin lymphoma, accounting for ~32 % of all NHL cases. It is an aggressive, fast‑growing B‑cell malignancy that usually responds well to frontline combination chemo‑immunotherapy (R‑CHOP). Relapsed or refractory disease is treated with CAR‑T, polatuzumab‑vedotin, tafasitamab, or other immunochemotherapeutic combinations. The prognosis is defined by the International Prognostic Index (IPI) and ranges from excellent (~90 % 5‑year OS) for low‑risk patients to poor (~30 % 5‑year OS) for high‑risk patients.

Early identification and aggressive treatment of DLBCL improve survival outcomes; timely use of R‑CHOP has a ~60 % complete remission rate. Relapsed disease requires high‑efficacy therapies such as CAR‑T or antibody‑drug conjugates. Understanding prognostic factors (IPI) guides therapy intensity and surveillance intensity to reduce morbidity and improve quality of life.
