Before the use of chemotherapy (which began in the 1970s), osteosarcoma was treated primarily with surgical resection (usually amputation).

Chemotherapy is essential for systemic control because most patients present with micrometastatic disease; surgical resection alone provides good local control but is insufficient to prevent widespread recurrence.
