Monitoring soluble cMET and ctDNA in metastatic uveal melanoma patients to track early disease progression on immunotherapies
Transpupillary thermotherapy (TTT) is used for selected small choroidal melanocytic tumours, either as primary treatment or as an adjunct to plaque brachytherapy.
This retrospective cohort study evaluated local recurrence and mortality outcomes following transpupillary thermotherapy (TTT) alone, TTT combined with plaque brachytherapy (brachy+TTT), and plaque brachytherapy alone in patients with small choroidal melanomas. At 15 years, Kaplan–Meier local recurrence‑free survival was 8% after TTT alone, 34% after brachy+TTT, and 86% after brachy alone. Disease‑specific survival at 15 years was 46%, 85%, and 90% respectively. Multivariable Fine–Gray regression demonstrated that primary TTT was associated with a higher subdistribution hazard of local recurrence (sHR, 6.7) and melanoma‑related death (sHR, 3.9). Subsequent recurrences after TTT were more frequent than after brachytherapy; the risk of extraocular extension increased with the number of TTT sessions.
Determining the comparative effectiveness of TTT versus plaque brachytherapy informs clinical decision‑making regarding eye‑preserving therapies for small uveal melanomas and helps identify treatment modalities that minimize local recurrence and melanoma‑related mortality.
