First-Line Odronextamab Shows High Complete Response Rate in DLBCL
Odronextamab with chemotherapy may be an effective intervention among some patients with previously untreated diffuse large B‑cell lymphoma (DLBCL), according to research presented at the ASCO Annual Meeting 2026.
In a dose‑optimization phase of the phase 3 OLYMPIA‑3 trial (NCT06091865), 40 patients with untreated CD20‑positive diffuse large B‑cell lymphoma received either odronextamab 160 mg followed by 320 mg (regimen 1) or odronextamab 160 mg alone (regimen 2) alongside CHOP chemotherapy over six 21‑day cycles. The overall response rate was 95% for regimen 1 versus 90% for regimen 2, with complete response rates of 85% in both groups. Progression‑free survival was not yet reached, and grade 3/4 adverse events were reported in 100% of patients overall. Despite the high rate of adverse events, the safety profile was described as manageable.
These findings suggest that adding odronextamab to standard CHOP chemotherapy could enhance complete response rates in previously untreated diffuse large B‑cell lymphoma, offering a potential improvement over current first‑line therapy but raising concerns about toxicity.
