Outcomes following ibrutinib to zanubrutinib transition in WaldenströM macroglobulinemia
The findings demonstrate that patients with WM who tolerated ibrutinib may safely transition to zanubrutinib without compromising efficacy.
The study evaluated 47 WaldenströM macroglobulinemia patients who switched from ibrutinib (phase III ASPEN) to zanubrutinib (long‑term extension 1). Primary outcome was safety; median follow‑up 15.3 months. 85 % remained on zanubrutinib, Grade ≥ 3 and serious treatment‑emergent adverse events (TEAEs) occurred in 23.4 % and 12.8 % respectively, with no new hypertension or worsening atrial fibrillation/flutter. Best overall response (BOR) was seen in 96 % (44/46).
This provides evidence that a clinically effective transition from one BTK inhibitor to another is feasible and maintains efficacy while keeping safety acceptable.
