SWOG study shows strong long-term survival rates for patients with GIST

Cancer research · Gastrointestinal stromal tumor

The PDGFRα antibody olaratumab showed activity in cell line xenograft models and an initial phase II trial, but failed to improve survival in a definitive phase III STS trial.

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors originating from interstitial cells of Cajal, with the stomach being the most common site of occurrence. These tumors have unique molecular characteristics, typically driven by mutations in the KIT or platelet-derived growth factor receptor alpha (PDGFRA) genes, leading to constitutive activation of tyrosine kinase pathways. GISTs exhibit diverse clinical behaviors, from indolent growth to aggressive metastasis, posing diagnostic and therapeutic challenges. Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) and immunohistochemistry (IHC), including markers like CD117 (KIT) and DOG1, are essential for accurate diagnosis and risk stratification. Surgical resection remains the primary treatment for localized GISTs, with laparoscopic and robotic-assisted techniques offering minimally invasive alternatives. In high-risk cases, adjuvant therapy with tyrosine kinase inhibitors (TKIs) like imatinib has significantly improved recurrence-free survival. For advanced or unresectable tumors, neoadjuvant imatinib therapy enhances resectability, while its use as an adjuvant consolidates postoperative outcomes. Additionally, systemic treatments, including second- and third-line TKIs, are crucial in managing metastatic disease. Our retrospective study of gastric GIST patients highlighted excellent postoperative outcomes with targeted therapy, underscoring the importance of a multidisciplinary approach in optimizing prognosis and quality of life for GIST patients.

The findings indicate cabozantinib could become an alternative therapy for GIST patients who have progressed on standard TKIs, potentially overcoming MET‑mediated resistance and improving clinical outcomes.
