Adult Wilms tumor revealed after radical nephrectomy for a renal mass initially suspected to be clear‑cell renal cell carcinoma: a case report

Adult Wilms tumor was the correct diagnosis and the patient achieved disease‑free status after multimodal therapy guided by pediatric protocols.

We report the case of an adult patient who was preoperatively suspected of having clear cell renal cell carcinoma and subsequently underwent a radical nephrectomy. Post‑operative histopathology confirmed nephroblastoma (Wilms tumor). The patient received the Children’s Oncology Group (COG) EE‑4A chemotherapy regimen. At 1 year post‑treatment, the patient was asymptomatic with no evidence of disease on imaging, indicating a favorable response. This case highlights the diagnostic challenge of adult Wilms tumor, which is often mis‑diagnosed as renal cell carcinoma. Treatment was guided by pediatric protocols, underscoring the need for adult‑specific guidelines.

Adult Wilms tumor is exceedingly rare; misdiagnosis can lead to inappropriate treatment. Successful multimodal therapy based on pediatric protocols demonstrates that adults can achieve favorable outcomes with timely, evidence‑based management.
