This review summarizes randomized and phase‑II trials that evaluate PET‑guided de‑escalation and escalation in advanced‑stage classical Hodgkin lymphoma (AS‑cHL). It highlights that early PET negativity allows safe reduction of chemotherapy intensity and that PET positivity guides intensification with BEACOPP or ASCT. The evidence supports PET‑adapted algorithms that maintain high progression‑free survival (>85 %) while lowering cumulative toxicity.
PET‑adapted strategies enable personalization of therapy, preventing overtreatment and long‑term sequelae in otherwise curable disease.
