The study developed and validated a five‑gene signature (SPP1, IL18BP, PALMD, WDR35, SOCS6) that predicts 3‑year progression‑free survival (PFS) and 5‑year overall survival (OS) in patients with locally advanced nasopharyngeal carcinoma (NPC). It was derived from a retrospective biomarker investigation using transcriptomic profiling and survival analyses in a discovery cohort from the multicenter, randomized phase III trial NPC‑0501 (N = 99) and validated in independent cohorts (N = 133) from Queen Elizabeth Hospital and Sun Yat‑sen University Cancer Center. The signature outperformed an existing metastasis‑related model and identified M2 macrophage enrichment (SPP1) as a potential mechanism of poor prognosis.
The five‑gene signature offers a robust, cross‑platform prognostic tool that can stratify patients into high‑ and low‑risk groups, enabling personalized treatment decisions and improving clinical outcomes in locally advanced NPC.
