Clinical Outcomes of Younger Adults With Philadelphia-Negative Acute Lymphoblastic Leukemia, Treated With A Pediatric-Like Regimen Based on the GIMEMA LAL1308 Protocol. A Campus ALL Study

Rákkutatás · Acute lymphoblastic leukemia

The 5‑year overall, disease‑free, and event‑free survival rates were 76.4 %, 69.1 %, and 59 %, respectively.

This retrospective multicenter study evaluated 36 adolescents and young adults (AYA) with Philadelphia‑negative acute lymphoblastic leukemia (Ph‑ALL) or lymphoblastic lymphoma (LL) who received a frontline pediatric‑like chemotherapy regimen (GIMEMA LAL1308) in real‑life clinical practice. Morphologic or radiologic/metabolic complete remission after induction Phase Ib was achieved in 86.1 % of patients. Measurable residual disease (MRD) negativity at the end of induction was attained in 73.9 % of evaluable ALL patients. Twenty‑five patients underwent allogeneic hematopoietic stem cell transplantation (allo‑HSCT). Five‑year overall survival, disease‑free survival, and event‑free survival were 76.4 %, 69.1 %, and 59 %, respectively, with no significant difference observed according to diagnosis, lineage, risk stratification, allo‑HSCT, or MRD status.

The findings indicate that an unmodified pediatric‑like regimen can be feasibly applied to Ph‑negative AYA patients in routine practice, yielding survival outcomes comparable to, or better than, other pediatric‑based protocols that do not yet include frontline immunotherapy. The study underscores the potential of risk‑adapted, MRD‑driven intensification strategies in this population, especially given the limited data on real‑world application of such regimens outside clinical trials.
