Taletrectinib in ROS1+ Non-Small Cell Lung Cancer: TRUST

Taletrectinib demonstrates high response rates and durable responses with favorable safety and low neurologic adverse events in ROS1+ non‑small cell lung cancer.

In the phase II, single‑arm TRUST‑I and TRUST‑II studies, 273 ROS1+ NSCLC patients received taletrectinib 600 mg once daily. TKI‑naïve patients experienced a confirmed objective response rate (cORR) of 88.8% (IC‑cORR 76.5%) and a median progression‑free survival (PFS) of 45.6 months. In TKI‑pretreated patients, cORR was 55.8% (IC‑cORR 65.6%) with median PFS of 9.7 months. The drug was well tolerated, with common grade 1–2 gastrointestinal events and low rates of central‑nervous‑system adverse events. The data demonstrate durable intracranial activity and prolonged disease control in a previously under‑treated population.

ROS1 rearrangements occur in a subset of NSCLC patients and represent a targetable oncogene. This study provides evidence that taletrectinib, a next‑generation ROS1 inhibitor, achieves high systemic and CNS response rates, thereby expanding therapeutic options for patients with both treatment‑naïve and pretreated disease, especially those with brain metastases.
