Biology of Advanced Uveal Melanoma and Next Steps for Clinical Therapeutics

Uveal melanoma is the most common intra‑ocular malignancy, yet it is a rare subset of all melanomas. It has distinct biology compared with cutaneous melanoma, leading to wide‐ranging patient outcomes. Approximately half of patients with a primary uveal melanoma eventually develop metastatic disease, predominantly to the liver, over a 15‑year period. Historically, no systemic therapy has shown a clear clinical benefit for those with advanced disease, and median survival remains poor. This review summarizes current understanding of the molecular and immunobiology of uveal melanoma, ongoing laboratory research, and recent investigations of targeted and immunotherapeutic approaches, including the clinical benefit seen with MEK inhibition.
Uveal melanoma’s distinct molecular profile and poor response to systemic therapy underscore the urgent need for effective, targeted treatments.
