Radiotherapy-free pembrolizumab combined with chemotherapy for locally advanced non-small-cell lung cancer with PD‑L1 tumour proportion score of 50% or higher (Evolution trial): a multicentre, single‑arm, phase 2 study
Pembrolizumab combined with platinum‑based chemotherapy, without radiotherapy, may provide a feasible and promising alternative treatment strategy for patients with unresectable, locally advanced NSCLC with a PD‑L1 TPS of 50% or higher.
This prospective, multicentre, single‑arm phase 2 study evaluated a radiotherapy‑free regimen of pembrolizumab plus platinum‑based chemotherapy in 21 patients with unresectable, locally advanced NSCLC with PD‑L1 TPS≥50%. Median follow‑up was 32.8 months; the 2‑year progression‑free survival rate was 67% (90% CI 46–83%). Common grade ≥ 3 adverse events were neutropenia, leukopenia and pneumonia. The results suggest that this combination is a feasible alternative to the standard chemoradiotherapy with durvalumab.
The study offers a radiation‑free therapeutic option that could reduce treatment‑related morbidity while maintaining efficacy in a patient population with high PD‑L1 expression.
