Induction Paclitaxel-Cisplatin-Capecitabine for Nasopharyngeal Carcinoma

5-year FFS was 77.6% in the TPC group and 62.9% in the PF group

In a multicenter, randomized trial, high‑risk locoregionally advanced nasopharyngeal carcinoma (LA‑NPC) patients received two 21‑day cycles of induction chemotherapy with TPC (paclitaxel, cisplatin, capecitabine) or PF (cisplatin, fluorouracil). The primary endpoint was failure‑free survival (FFS). After a median follow‑up of 89.1 months, 5‑year FFS was 77.6% in the TPC group and 62.9% in the PF group (HR 0.52, 95% CI 0.34‑0.82). 5‑year distant‑metastasis‑free survival, locoregional‑relapse‑free survival, and overall survival were also higher in the TPC arm. Grade 3/4 adverse events occurred in 57.6% of TPC patients and 65.8% of PF patients.

The study demonstrates that induction paclitaxel‑cisplatin‑capecitabine improves 5‑year failure‑free survival compared with cisplatin‑fluorouracil in high‑risk locoregionally advanced nasopharyngeal carcinoma, supporting the use of TPC as a more effective induction regimen.
