Neoadjuvant chemotherapy for resectable non‑small‑cell lung cancer (NSCLC) is a strategy that may reduce tumour volume, eradicate micrometastatic disease early and improve outcomes. Randomised trials comparing neoadjuvant platinum‑based regimens with surgery alone have demonstrated feasibility and safety of this modality, supporting evidence from phase II trials that utilised third‑generation chemotherapies. Despite these studies, limitations exist, including heterogeneous disease stages, variable chemotherapy regimens and a lack of phase III data directly comparing neoadjuvant with adjuvant chemotherapy. These factors make it difficult to provide firm recommendations about neoadjuvant chemotherapy. Other areas of contention involve postoperative radiation and the possibility of increased postoperative complications, especially after a right pneumonectomy following neoadjuvant chemotherapy.
The evidence regarding the benefit, safety and optimal regimen for neoadjuvant chemotherapy in resectable NSCLC remains uncertain. Clarifying its role is essential for improving long‑term survival and guiding surgical decision‑making.
