The rate of colorectal cancer is rising among adults younger than 50

While rates in older adults have fallen due to screening and reduced smoking, epidemiologic data show a rising incidence among younger adults, especially among Alaska Native, American Indian, White, and to a lesser extent Black populations. The underlying causes remain poorly defined, with lifestyle factors such as obesity, diet high in processed meats, physical inactivity, and potential gut microbiome alterations implicated. Genetic predispositions, notably Lynch syndrome and familial adenomatous polyposis, account for roughly 10‑20% of early-onset cases. Environmental exposures to endocrine‑disrupting chemicals, obesogens, and pollutants are also considered potential contributors. The article emphasizes the importance of early detection, increased awareness of warning signs, and the need for research to clarify etiologic drivers and develop targeted prevention and treatment strategies.

Early‑onset colorectal cancer carries a higher mortality risk due to later symptom recognition and limited screening guidelines for younger adults. Understanding the drivers of this trend is crucial to improve prevention, refine screening recommendations, and develop interventions that reduce the incidence and mortality in this growing patient population.
