Treatment Patterns and Survival Outcomes Among Patients With Hepatocellular Carcinoma
Atezolizumab‑bevacizumab and durvalumab‑tremelimumab replaced sorafenib as the most common first‑line therapy over time, and only about 20% of patients received second‑line therapy; median overall survival was 8.1 months and did not differ significantly by first‑line treatment.
This retrospective cohort study used a nationwide electronic health record database to evaluate systemic therapy patterns and outcomes among 4,198 patients with hepatocellular carcinoma (HCC) treated between 2011 and 2023. The analyses revealed that atezolizumab‑bevacizumab and durvalumab‑tremelimumab largely replaced sorafenib as most common first‑line regimens by 2020–2023, yet only 20% of the cohort proceeded to second‑line therapy. Median overall survival was 8.1 months, regardless of first‑line therapy type, while the combination of atezolizumab‑bevacizumab was associated with modestly improved progression‑free survival compared with sorafenib.
Accurately mapping real‑world treatment patterns and survival outcomes in HCC guides clinicians on sequencing therapies, identifies gaps in second‑line utilization, and informs future comparative studies.
