Encorafenib, Binimetinib, and Cetuximab in BRAF V600E‑Mutated Colorectal Cancer

A combination of encorafenib, cetuximab, and binimetinib significantly improves overall survival and response rates compared with standard therapy in metastatic colorectal cancer patients with the BRAF V600E mutation.

In this Phase 3, open‑label, randomized controlled trial, 665 patients with metastatic BRAF V600E‑mutated colorectal cancer who had progressed after one or two prior regimens were randomized to (1) encorafenib, binimetinib, and cetuximab (triplet), (2) encorafenib and cetuximab (doublet), or (3) standard therapy (cetuximab with irinotecan or FOLFIRI). The triplet group achieved a median overall survival of 9.0 months versus 5.4 months in the control group (HR 0.52; 95% CI 0.39–0.70). Objective response rate was 26% versus 2% (P < 0.001). Adverse events of grade 3 or higher were reported in 58% of the triplet arm.

The study establishes a new standard of care for metastatic colorectal cancer harboring BRAF V600E, substantially improving survival and response rates over existing therapies.
