On the architecture of translational research designed to control chronic lymphocytic leukemia
Recent advances in the biology of CLL and the development of targeted therapeutics such as BCR‑signaling inhibitors and anti‑CD20 monoclonal antibodies have dramatically improved clinical outcomes and may enable long‑term remission or cure in selected patient populations.
Chronic lymphocytic leukemia (CLL) has been one of the most dynamic fields of clinical research over the last two decades. Important advances in understanding the biology of CLL have led to the development of new prognostic and diagnostic tools. Concurrently, several recently approved new agents hold the potential to fundamentally change the management of this leukemia and have started to improve clinical outcomes for patients. This conceptual review summarizes the major recent insights regarding the biology of CLL, the technological advances that have allowed refinement of the prognostication of the clinical course, and the new therapeutic strategies that are currently under investigation to further ameliorate the outcome for patients with CLL.
CLL was historically an incurable disease with poor survival; the advent of targeted agents has transformed standard of care, reduced time to progression, and opened possibilities for remission and potentially cure. Understanding these translational advances is essential for clinicians, researchers, and patients to optimize treatment decisions and guide future innovation.
