Tumor response to immunochemotherapy may spare some head and neck cancer patients radical surgery
In patients with HPV‑negative locally advanced resectable head and neck squamous cell carcinoma (HNSCC), neoadjuvant immunochemotherapy (ICT) induced ≥50 % tumor regression in 90 % of 47 evaluable patients, allowing reduced‑volume surgery, resulting in a 2‑year disease‑free survival of 90 % and complete preservation of laryngeal function. Higher PD‑L1 Combined Positive Score (>20) was associated with 65 % pathological complete response.
This study demonstrates that response‑adapted surgery after neoadjuvant ICT can provide excellent disease control and organ preservation in HPV‑negative HNSCC, informing future trials that may transform treatment paradigms for this high‑risk patient group.
