This article reviews the current status of neoadjuvant therapy for locally advanced renal cell carcinoma, summarizing the disappointing results of existing trials and underscoring the need for larger, adaptive randomized studies.

Optimizing neoadjuvant regimens could reduce tumor burden before surgery, improve surgical outcomes, and enhance long‑term survival for high‑risk renal cell carcinoma patients.
