What does cancer stage mean, and why is the TNM classification important?
Cancer staging refers to the determination of the extent of neoplastic disease in the body at the time of diagnosis or surgery. The most widely used international system is the TNM system maintained by the UICC and the AJCC. The acronym denotes three fundamental anatomical factors: T (Tumour, the size of the primary tumour and its extent of invasion into immediately adjacent structures), N (Node, the involvement of regional lymph nodes), and M (Metastasis, the presence or absence of distant metastases). The TNM classification is of paramount importance because it provides a shared international professional language for clinicians, fundamentally determines the therapeutic strategy (such as resectability, or the need for neoadjuvant or adjuvant oncological treatments), provides prognostic information regarding the expected outcome, and enables the standardised comparison of clinical trial results and registry data.\n\nOverall stage groups (typically from 0 to IV) are derived from the combination of TNM categories, distinguishing early, localised processes (I–II) from locally advanced or lymph node-positive (III) and distant metastatic (IV) diseases. In addition to anatomical extent, modern guidelines (such as the 8th and 9th editions of the AJCC and UICC) increasingly integrate supplementary pathological and molecular prognostic factors as well.
